By agreeing to provide us with a testimonial, you authorize Teladoc Health, Inc., and its subsidiaries including but not limited to Best Doctors and Advance Medical to use and publicly display your testimonial in whatever form (e.g. video, photo, and/or text) for marketing, advertising, sales, promotional and other similar purposes.
If Teladoc Health decides to publish your testimonial, we will use exactly what you write, except as edited to fix spelling and grammatical errors and shorten the length, if necessary. Therefore, to the extent you include in your testimonial any health information or the provision of certain types of services by Teladoc Health (such as Expert Medical Opinion, behavioural health or general medicine), it will be published and disclosed publicly.
You understand and agree that use of your testimonial means it will be re-disclosed and available publicly and may no longer be protected by privacy laws.
This authorization shall continue until you revoke it at any time by notifying Teladoc Health in writing by (a) sending a letter addressed to: The Data Protection Officer, Calle Almagro 36, 28010 Madrid, Spain or by (b) sending an email to:
[email protected]. Your revocation will become effective upon receipt by Teladoc Health, except to the extent that Teladoc Health has already acted in reliance upon this authorization. You understand and agree that even after your revocation there is no way for Teladoc Health to undisclose any previously published information and it will remain in the public domain.
Teladoc Health will not refuse or condition your treatment, enrolment, or eligibility for services provided by Teladoc Health upon you providing this authorization.
Teladoc Health will own all right, title, and interest in and to your testimonial and Teladoc Health shall have the exclusive right to reproduce, edit, display, broadcast, distribute, license, and otherwise use your testimonial. You will not receive any fee, royalties or other reimbursement for the rights granted herein.
